Job summary

Hackensack Meridian Health in Iselin, NJ is seeking a Registered Nurse for Home Care to provide professional nursing care to patients at home, following physician orders and developing individualized plans.

The role offers a full-time schedule, with opportunities to collaborate with physicians, supervise home health aides, and stay current with new standards while meeting productivity requirements.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing is required.
• At least 6 months RN experience or 1 year LPN experience, preferably in home care or hospice.
• Current NJ State Professional Registered Nurse License.
• AHA Basic Health Care Life Support HCP Certification.

Responsibilities

• Provide professional nursing care to patients in their home.
• Assess, develop, implement, and evaluate home nursing care needs.
• Ensure compliance with physician orders and documentation.
• Demonstrate holistic care for patients and their significant others.
• Complete nursing assessments and medication reconciliations.
• Collaborate with physicians on the Plan of Care.
• Act as a liaison among physicians, management, and healthcare team members.
• Supervise para-professional home health aide staff.
• Maintain awareness of new standards and incorporate them into practice.
• Complete routine assignments independently and meet productivity requirements.
