Job summary

Senior LIFE PACE is expanding our RN Home Care/Center team in Johnstown. Our RNs work as an extension of our Clinic Team with the autonomy to provide care in home and community settings for the senior population in Johnstown and surrounding areas.

You will deliver one-on-one care to enable participants to live independently at home, building meaningful relationships with participants, families, and team members.

Responsibilities

• Provide meaningful one-on-one care to homebound participants.
• Coordinate with Clinic Team while maintaining autonomy to deliver care in home and community settings.
• Support participants and their families, fostering relationships and continuity of care.
