Job summary

Expect two-week orientation, strong collaboration with clinicians, and travel to patient homes in the county. A bachelor’s degree in nursing is preferred, with 1+ year RN experience and proficiency in EMR systems.

Qualifications

• RN with a bachelor’s degree in nursing; Bachelor’s preferred.
• 1+ year of clinical experience as an RN; homecare experience a plus.
• Strong verbal and written communication for patient/family education.
• Proficient with EMR systems and digital tools; reliable transportation.

Responsibilities

• Assess patients with various health needs and develop personalized care plans.
• Educate patients and families to manage health conditions at home.
• Collaborate with VNA clinicians and external providers for seamless care.
• Document changes in patient conditions and update plans accordingly.
• Provide compassionate, high-quality care and advocacy for patients.
