Smith Arnold is hiring a Home Care Registered Nurse Case Manager in Poughkeepsie, NY. This full-time role delivers in-home nursing to geriatric patients, handling start-of-care admissions and follow-up visits while leveraging the system's resources.

You will perform skilled nursing tasks (wound vacs, drains, INR draws, dressing changes), educate families, maintain EMR notes, and collaborate with the interdisciplinary home care team.

Qualifications

• ADN required and NY RN license active.
• 1+ year of RN experience preferred, home care preferred.
• BLS certification required.

Responsibilities

• Provide in-home care to geriatric patients with start-of-care visits and follow-ups.
• Perform skilled nursing tasks such as wound vacs, drains, INR draws, and dressings.
• Educate patients and families about conditions and care plans.
• Document notes in EMR and collaborate with the home care team.

Location: Poughkeepsie, NY

Compensation: $100,000 - $125,000 + large sign-on bonus

Responsibilities for Home Care RN Case Manager:

• Provide in-home care primarily to geriatric patients
• Perform skilled nursing services including wound vacs, PleurX drains, INR draws, dressing changes, and more
• Conduct start-of-care admission visits and re-visits
• Educate and communicate with patients and family members regarding medical conditions and care plans
• Manage a manageable daily caseload of 4-5 patients per dayDocument clinical notes accurately within the EMR system
• Collaborate closely with interdisciplinary home care team members

Preferred Qualifications for Home Care RN Case Manager:

• Associate Degree in Nursing (ADN)
• 1+ year of Registered Nurse experience
• Active New York State RN license
• BLS certification
• Prior home care experience strongly preferred
