Job summary

AmeriCare Medical is seeking experienced Registered Nurses to join our home health team in the Sterling Heights area. This contingent, visit-based role offers flexibility, autonomy, and the opportunity to deliver compassionate care in patients' homes.

Qualifications

• Active RN license in Michigan is required.
• Minimum of 1 year nursing experience.
• Proficiency with electronic documentation systems is preferred.

Responsibilities

• Conduct Start of Care (SOC) assessments and admissions.
• Complete supervisory visits to ensure quality of care and regulatory compliance.
• Develop, review, and coordinate patient care plans.
• Perform skilled nursing visits as needed.
• Provide wound care and wound vac management.
• Provide tracheostomy (trach) care and patient education.
• Collaborate with physicians, therapists, caregivers, and interdisciplinary team members.
• Educate patients and families on disease management, medications, and care plans.
• Maintain accurate documentation in the EMR.
• Ensure compliance with agency policies and nursing standards.

Skills

Key Responsibilities

Qualifications

Required:

• Active Registered Nurse (RN) license in the State of Michigan
• Minimum of one year of nursing experience
• Valid driver's license and reliable transportation
• Ability to work independently and manage a flexible schedule
• Strong assessment, communication, and organizational skills
• Proficiency with electronic documentation systems
• Ability to pass a criminal background check
• Authorization to work in the United States

Preferred Qualifications

• Home health experience
• Experience conducting Start of Care (SOC) assessments
• Experience performing supervisory visits
• Wound care and wound vac experience
