Trinity Health Of New England At Home is seeking a Home Care RN in the West Hartford region. The role involves providing primary nursing care in patients' homes, assessing and planning care with physicians, and supervising aides as needed.
Position offers a Monday–Friday 8am–5pm schedule, one weekend every 4 weeks, and two rotating holidays with no on-call requirements. A $10,000 sign-on bonus is paid out over two years.

Our Home Care RN’s provide primary nursing care to home based care patients as ordered by the physician, and in accordance to the organization’s policies and procedures. The RN is responsible for the initial assessment and care plan development in collaboration with the Population Health Case Manager. They also provide functional support/supervision to Home Healthcare Aides and LPNs as needed and therapeutic intervention, overall care management and achievement of top decile client outcomes and patient satisfaction.
Schedule: Monday - Friday, 8 am - 5 pm, one weekend every 4 weeks and two rotating holidays per year. NO ON CALL REQUIREMENTS!

This position is 100% home care providing services in patients’ private homes.
