Job Description

Performs an age-specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation, and evaluation of patient care. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills and clinical judgment in coordinating patient care and directing/delegating activities of the patient care unit team.

Please note: The 'minimum' and 'maximum' rates refer to a 'per-visit' rate for regular assignments in this role. Voluntary work opportunities, outside the normal scope of duty, may pay an alternate ‘per-visit' rate, depending on multiple factors (region, type of visit, discipline, etc.), and may be offered on an occasional, as-needed basis.

The following rates are paid for certain administrative tasks: Orientation: 1/2 Day = $75.00; Full day = $150.00; Education/In-services: $40.00 - $150.00; Meetings: $35.00

Job Responsibilities
  1. Conduct patient interviews, explain policies and procedures, review patient charts, and assess health status across various systems; determine mobility, sensory deficits, skin condition, and pain levels; document and communicate findings.
  2. Assess psychosocial health, determine coping mechanisms, cultural needs, and develop care plans including patient education and support services.
  3. Develop patient outcome statements, set goals, prioritize care activities, and coordinate resource use; document and collaborate with the healthcare team; ensure safety and discharge planning.
  4. Administer treatments, monitor patient responses, provide emotional support, and ensure accurate documentation; respond effectively during emergencies; communicate with families and coordinate referrals.
  5. Evaluate care effectiveness, perform variance analysis, and contribute to quality improvement initiatives; document outcomes and collaborate on treatment plans.
  6. Interpret and implement treatment regimens, assist with procedures, monitor patient condition, and delegate tasks appropriately; perform skin care and medication administration following protocols.
  7. Educate patients and families about medications, conditions, and post-discharge care; document educational interactions and assess teaching effectiveness.
  8. Perform related duties as required, ensuring compliance with ADA and other relevant standards.
Job Qualifications

Graduate from an accredited School of Nursing; Bachelor’s Degree in Nursing preferred. Must be enrolled in an accredited BSN program within two years and obtain a BSN within five years of employment. Current RN license in New York State and relevant certifications required. Additional salary details depend on various factors including location, experience, and credentials.
