CONSUMER DIRECTED SERVICES INC. is seeking a Registered Nurse for Home Care Hospice & Palliative care. You will assess patients, develop individualized care plans, administer medications, and provide wound and pain management in patients' homes.

Collaborating with an interdisciplinary team, you will educate families, maintain documentation, and participate in on-call rotations to ensure compassionate, patient-centered end-of-life care.

Provide compassionate, skilled nursing care in patients’ homes, focusing on comfort, symptom management, and quality of life at end of life. Conduct comprehensive assessments and develop individualized care plans. Administer medications, manage wounds, and support effective pain control. Coordinate with an interdisciplinary team to ensure seamless, holistic, person-centered care.
