Job summary

Yale New Haven Health (YNHHS) is seeking a Home Health Physical Therapist to evaluate, plan and deliver therapy in patients' homes under physician direction in CT. This role requires a Connecticut PT license, a valid driver's license, CPR certification, and the ability to travel between stops daily.

The clinician will supervise PTAs and home health aides, coordinate care with the team, document EMR notes within 24 hours after visits, and maintain productivity and safety standards.

Qualifications

• Bachelor of Science in Physical Therapy required.
• Minimum of two (2) years of clinical experience in Physical Therapy in an acute or sub-acute care setting; home health preferred.
• Connecticut license for Physical Therapy; valid driver's license and proof of car insurance required when using a personal vehicle for business.
• CPR certification required; must be maintained; obtain within 3 months (90 days) of hire if not current.
• Excellent written and verbal communication; ability to supervise PTAs; strong computer skills.

Responsibilities

• Comprehensive evaluation of patient function and development of the therapy plan of care.

• Instruction of patients, caregivers, PTAs, and other staff in the treatment plan and follow-up supervision.
• Selects and administers diagnostic and therapeutic techniques and materials.

• Supervision of Home Health Aides when participating in the patient's care plan.
• Documentation in EMR within 24 hours after visits.
• Assist patients in obtaining equipment and instruct on use.
• Maintain productivity and communicate needs for additional visits.
• Ensure insurance authorization prior to visits where appropriate.
• Report incidents to RN case manager and Therapy Services Manager.
• Participates in staff meetings and other requests.
• Coordinates and manages care of patients in home settings.

Responsibilities

• 1. Comprehensive evaluation of the patient's level of function and development of the therapy plan of care.
• 2. Participation in case management conferences and documentation thereof.
• 3. Instruction of patient, caregivers, Physical Therapy Assistants (PTAs), and other agency clinical staff in the patient's treatment plan, including follow-up supervision.

• 5. Supervision of PTAs. Confers with PTAs at least weekly and when there is a change in condition.
• 6. Supervision of Home Health Aides when they are participating in the patient's therapy care plan.
• 7. Documentation is completed in the EMR within 24 hours after visits are made.
• 8. Assists patients and caregivers in obtaining necessary equipment and provides instruction on how to use equipment.
• 9. Responsible for maintaining productivity and appropriately communicates needs for additional visits when necessary.
• 10. Ensures that there is insurance authorization prior to making visits when appropriate.
• 11. Reports incidents that occur to the RN case manager and the Manager, Therapy Services.
• 12. Participates in staff meetings, in-service programs, and other meetings as requested.
• 13. Coordinates and manages the care of patients receiving services in their home.
• 13.1 Participates in case management conferences and documents per guidelines.
• 14. May perform other duties as assigned.

Qualifications

EDUCATION

Bachelor of Science in Physical Therapy required, masters or doctorate desirable.

EXPERIENCE

Minimum of two (2) years of clinical experience in Physical Therapy in an acute or sub-acute care setting. Home care experience is preferred.

LICENSURE

Physical Therapist with valid (or eligible) Connecticut license. A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day. CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire.

SPECIAL SKILLS

Excellent written and verbal communication. Ability to work collaboratively with multidisciplines. Must be adaptable, flexible, able to learn quickly, use critical thinking skills, and oversee PTAs. Strong computer skills.

PHYSICAL DEMAND

Ability to perform tasks involving physical activity, which includes heavy lifting, extensive bending, administering CPR, patient bracing, standing, and stairs. Requires exposure to blood and body fluids, hazardous materials, and communicable diseases.
