Job summary

Apex Rehabilitation in Sullivan County, Monticello area is seeking a Home Care Physical Therapist to deliver high-quality care in patients' homes. You will assess and implement therapy to improve mobility and independence while collaborating with families and other healthcare professionals.

This role offers flexible scheduling across full-time, part-time, PRN, or per diem options, with training support and a referral bonus.

Qualifications

• DPT from an accredited program is required.
• State licensure as a Physical Therapist is required.
• Experience in home care preferred; new graduates welcome.

Responsibilities

• Conduct comprehensive assessments of clients in a home care setting.
• Develop personalized, goal-oriented PT plans based on assessments.
• Administer therapeutic exercises and manual therapy to improve function.
• Collaborate with clients, families and other healthcare professionals.
• Educate clients on home exercises and self-care strategies.
• Maintain accurate documentation of assessments, interventions, and progress.
