Job summary

Montefiore Nyack Hospital in New City, NY is seeking a licensed Physical Therapist to provide in-home and clinical PT services with direct patient care, evaluation, and supervision responsibilities. The role supports care in the Home Care department and requires active collaboration with physicians and interdisciplinary teams.

Qualified candidates will have NYS licensure, BLS certification, and at least two years of staff PT experience (one year in home care preferred).

Qualifications

• Two years work experience as staff Physical Therapist and a minimum of one year in Home Care preferred.
• Physical Therapists - New York State Board of Physical Therapy.
• BLS - American Heart Association.

Responsibilities

• Provides physical therapy services: evaluation, direct patient care, teaching, supervision and consultation to patients in their homes.
• Develops, reviews, implements and revises the patient Plan of Care in collaboration with the patient/family, and under the direction of the physician; initiates physician orders.
• Coordinates and maintains effective communication with other disciplines: OT, ST, nursing, social work, home health aide; supervises home health aides in rehab duties.
• Maintains contact with managed care case managers for authorizations; acts as case manager for Rehabilitation Only patients.
• Maintains comprehensive medical records and communications in accordance with Home Care department policies.
