Excellent Home Care NY provides top-quality home health care services in the New York City Area. We are seeking a Physical Therapist to join our team and deliver therapy in patients' homes, evaluating function and developing plans of care in collaboration with families.

The PT will work with a multidisciplinary team, document progress, educate patients and families, and support ongoing professional growth within a supportive environment.

Qualifications

• Minimum one year of acute care Physical Therapy experience; two years preferred.
• Home care experience is desirable.
• Strong assessment and technical PT skills.

Responsibilities

• Plan, provide and coordinate Physical Therapy to agency patients.
• Evaluate patients and work with family to set goals and provide training.
• Collaborate with health team to establish rehab goals and activities.
• Perform clinical tests to assess function and plan care.
• Instruct patients, families, and caregivers in PT procedures.
• Re-evaluate progress and document outcomes and care plans.
