Job summary

Personal Touch Home Care is seeking a licensed Occupational Therapist to join our fee-for-service home-care team in New York. You will assess patients, develop and implement treatment plans, and help clients improve daily living and work skills in the comfort of their own homes.

This 1099 per-visit position offers flexible daytime and after-school/evening hours, with organized caseloads and supportive leadership.

Qualifications

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Demonstrates good verbal and written communication and organization skills.
• Prefer two (2) years of appropriate experience as an occupational therapist. Home health experience is preferred.

Responsibilities

• Responsible for the assessment of referred patients that require rehabilitation services.
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients.
• Document and record the patients' condition.
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborates with all disciplines to provide the best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation.
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
