Job summary

Personal Touch Home Care in Bronx, NY seeks a compassionate Occupational Therapist (OT) to join our Special Needs Program in the home care setting. This is a fee-for-service, per-visit 1099 contracted role requiring a valid NY license.

The position offers flexible daytime and after-school/evening cases with organized caseloads to minimize travel. You will assess patients, develop and implement treatment plans, and help clients improve daily living and work skills while focusing on mobility and

Qualifications

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid NY license/registration.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Provide direct patient care and assess medical condition and functional limits.
• Develop and administer treatment plans with goals according to physician directions.
• Set realistic goals for patients.
• Document patient condition and progress.
• Educate patients and families in therapy methods.
• Maintain weekly progress notes and discharge documentation.
• Collaborate with disciplines for patient-centered care.
• Perform duties assigned by Director of Rehabilitation.
• Ensure confidentiality and HIPAA compliance.
