Job summary

Metro Physical & Aquatic Therapy seeks a motivated per diem Occupational Therapist to provide in-home care around Port Washington. All levels of experience are welcome, with mentorship offered for documentation and compliance.

The role supports flexible scheduling and offers both 1099 and W-2 options to suit your preferred work style. Responsibilities include 1:1 treatment, evaluating patients, and creating individualized plans aligned with physician orders.

Qualifications

• Active Occupational Therapist license in New York.
• Home health experience preferred but not required.
• Ability to travel to patients' homes within territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care.
• Educate family members, home health aides, and other caregivers on proper therapy techniques.
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Must be willing to treat geriatric patients.

Qualifications

• Occupational Therapist Program Graduate
• Licensed Occupational Therapist in the state of New York or eligible.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory
• Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc.

Additional Information

The anticipated base hourly range for this position is $70-90. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
