Job summary

Metro Physical & Aquatic Therapy is seeking a per diem Occupational Therapist to deliver in-home care in Brooklyn. The role offers part-time hours, flexible scheduling, and 1099 or W-2 options. You will provide 1:1 therapy, develop plans, and educate caregivers in a supportive, professional environment.

Licensed NY OT with ability to travel to patients’ homes is required; new grads welcome and mentored. Pay ranges from $75 to $90 per visit, depending on experience and location.

Qualifications

• OT program graduate and licensed to practice in NY
• Must be able to travel to patients' homes within geographic territory
• Strong communication with patients, families, and care team

Responsibilities

• Accept and treat patients in designated geographic area
• Provide 1:1 treatment and evaluate needs
• Develop individualized treatment plans per physician orders
• Deliver care per standards and policies
• Educate family members and caregivers on therapy techniques
• Plan and carry out in-home occupational therapy programs
• Perform initial exam and documentation for each visit

Home Care Occupational Therapist- Per Diem (Brooklyn)

• Compensation: USD 75 - USD 90 - hourly

Responsibilities:

• Accept and treat patients within your designated geographic area
• Provide 1:1 treatment
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
• Educate family members, home health aides, and other caregivers on proper therapy techniques
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
• Must be willing to treat geriatric patients.

Qualifications

• Occupational Therapist Program Graduate
• Licensed Occupational Therapist in the state of New York or eligible.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory
• Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc.

The anticipated pay range for this position is$75-90 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
