Join to apply for the Home care Nurse (RN and LPN) role at Jethro Health System

We are seeking Part-time or Per Diem Registered Nurses (RNs) and LPNs to join our team! As a home care nurse, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to our clients, allowing them to receive medical care in their homes to maintain independence.

This position requires driving to assigned territories, including North Shore Areas, Middlesex County, and Essex County.

Responsibilities include:

  • Initiating and updating care plans with input from physicians and the healthcare team.
  • Conducting evaluations to ensure patient eligibility and medical necessity per payer guidelines.
  • Providing high-quality clinical services within scope and infection control standards, aligned with care plans.
  • Administering medications and treatments as prescribed, ensuring safety standards.
  • Monitoring patient progress and adjusting care plans as needed.
  • Educating and supporting patients and their families regarding health management and home care techniques.
  • Performing assessments to identify medical needs, functional levels, and communication abilities.
  • Initiating and revising care plans, performing OASIS assessments when applicable.
  • Coordinating services and informing physicians and other personnel of changes.
  • Collaborating with patients, families, and community resources for planning and discharge.
  • Preparing and submitting all required documentation timely.

Requirements:

  • Massachusetts RN license required.
  • Homecare/VNA experience preferred.
  • At least 1 year of experience as a Home Health RN.
  • Current RN license in good standing.
  • Current BLS certification.
  • Valid driver's license, vehicle insurance, and reliable transportation.
  • Commitment to high standards of patient care and safety.

Job Types: Full-time, Part-time, Per diem

Pay: $35.00 - $100.00 per hour

Expected hours: 10 – 40 per week
