Home Care Nurse - Halethorpe/Catonsville/Pikesville/Reisterstown - Sign on $5000

Join GBMC HealthCare as a Home Care Nurse serving the Halethorpe, Catonsville, Pikesville, and Reisterstown areas.

Job Overview

As part of the interdisciplinary team, the Hospice Nurse provides care to terminally ill patients and their families at home in the west side of Baltimore County. Responsibilities include assessing patient and family needs, providing nursing and supportive care, and reporting to the Team Manager.

Qualifications

• Diploma, ADN, and/or BSN from an accredited institution
• Current Maryland State license as a Registered Nurse
• Minimum of two years of nursing experience or equivalent education and experience
• Knowledge of infection control and safe nursing practices
• Strong time management and communication skills
• Proficiency with EMR systems or ability to learn

Responsibilities

1. Conduct initial assessments of patients and families to determine home care needs.
2. Develop and implement care plans collaboratively with the interdisciplinary team.
3. Provide direct nursing care to maintain the highest level of patient care.
4. Coordinate care delivery with Home Health Aides and other team members.
5. Educate families and caregivers on care skills and emotional support.
6. Ensure proper equipment arrangements and documentation.
7. Participate in care conferences and update the primary physician regularly.
8. Attend to patients, families, and facility needs during end-of-life care.

Additional Duties

• Maintain positive relationships with facility staff and document in facility charts.
• Reconcile medications and communicate visit schedules.
• Participate in care planning meetings and provide staff education.
• Engage in marketing and customer service activities.

Compensation and Benefits

Salary range: $64,992.56 - $99,876.12, based on experience and qualifications. Sign-on bonus of $5,000 is offered.
