Job summary

Redwood Coast PACE, operated with Humboldt Senior Resource Center, seeks a Home Care Manager (LVN or RN) to oversee in-home care services for PACE participants in Fortuna. You will supervise the CarePartner team, coordinate safety assessments and durable medical equipment, and collaborate with agencies and interdisciplinary teams to ensure high-quality, participant-centered care.

The role requires a California RN or LVN license, 2+ years of RN/LVN experience, and supervisory experience, with BSN

Qualifications

• 2+ years RN/LVN experience
• 1+ year of supervisory experience
• Experience with medically frail or elderly populations
• CPR and First Aid certification within 6 months of hire
• Bachelor of Science in Nursing preferred

Responsibilities

• Oversee day-to-day home care services for PACE participants
• Supervise Home Care Assistants and related roles
• Coordinate hiring, orientation, training, supervision, and performance management
• Oversee contracted home care agencies and in-home CarePartners
• Assess home care needs of frail elderly participants
• Conduct home safety visits as assigned
• Coordinate durable medical equipment and related services
• Ensure assessments are completed and documented accurately and on time
• Support quality assurance and performance improvement initiatives
• Address participant rights violations and quality-of-care concerns
• Provide on-call coverage in coordination with the Home Care team
• Collaborate with staff, contractors, and community partners to support PACE goals

Job Title: Home Care Manager (LVN or RN)

Salary Range: $43.27 - $52.88/hour

Schedule: Full-Time | Monday - Friday | 8:00 AM - 5:00 PM

Requirements to Apply

• Current California RN or LVN license
• At least 2 years of RN/LVN experience
• At least 1 year of experience working with medically frail or elderly populations
• At least 1 year of supervisory experience
• Personal vehicle with required insurance
• CPR and First Aid certification within 6 months of hire

Job Overview

The Home Care Manager oversees and coordinates home care services for PACE participants in the Fortuna area. Key Responsibilities

• Oversee day-to-day home care services for PACE participants
• Supervise Home Care Assistants, Home Care Schedulers, Home Care Supervisors, and Lead CarePartners
• Coordinate hiring, orientation, training, supervision, and performance management of team members
• Oversee contracted home care agencies and in-home CarePartners
• Assess home care needs of frail elderly participants
• Conduct home safety visits as assigned
• Coordinate durable medical equipment and related services

• Participate in the Redwood Coast PACE Management team
• Provide on-call coverage in coordination with the Home Care team

Qualifications

• BSN preferred
• 2+ years of RN/LVN experience required
• 1+ year working with medically frail or elderly populations required
• 1+ year of supervisory experience required
• California RN or LVN license required
• Valid California Driver's License and personal vehicle required
• CPR and First Aid certification required within 6 months of hire

Additional Requirements

• DOJ and FBI criminal history background clearance
• Pre-employment physical and PPD skin test
• Motor Vehicle Report verification
• COVID-19 and influenza vaccinations preferred
