Job summary

The Center for Elders’ Independence (CEI) is seeking a Licensed Vocational Nurse (LVN) to provide in-home nursing services in San Leandro, CA. Under RN supervision, you will follow care plans, perform assessments, administer medications, monitor vitals, and coordinate with the interdisciplinary team to support elderly participants in the community.

The role requires a CA LVN license, at least one year of acute care experience, and strong communication skills. A $2,000 sign-on bonus is offered.

Qualifications

• CA LVN license required.
• 1+ year acute care experience preferred.
• Experience with frail or elderly population preferred.
• Current CPR & First Aid certification.

Responsibilities

• Perform clinical assessments and vital signs.
• Monitor and document patient conditions in records.
• Administer medications and vaccines as prescribed.
• Coordinate with RN supervisor and care team.
• Educate patients and families on home care procedures.
• Maintain HIPAA compliance and accurate records.
