Job summary

Liberty Healthcare Management in Fayetteville, NC seeks an experienced LPN Office Nurse to provide direct patient care and coordinate services for home care patients. The role includes efficient documentation, and supporting the Care Coordinator and Operations Manager in daily branch tasks.

The position requires current LPN licensure in North Carolina, minimum one year of clinical nursing experience, and CPR certification.

Qualifications

• Current LPN licensure in the state of the branch location.
• One year of clinical nursing experience.
• One year of Home Care experience preferred.
• Ability to independently organize work and provide effective health care services to homebound patients.
• Effective communication skills (oral and written).
• CPR certified.

Responsibilities

• Provides direct patient care and coordinates services.
• Completes documentation as required by policy and regulation.
• Assists Patient Care Coordinator/Operations Manager in daily branch tasks to ensure timely documentation.
• Assists in management of patient care supplies.
• Manages commercial insurance payers and Medicare replacement plans for continued authorization of home care visits and documents authorization in the EMR.
• Coordinates Face to Face documentation with the manager and care coordinator as required by payer sources.
