Job summary

FlexStaff is seeking a Registered Nurse (RN) with home care (CHHA) experience to work full-time as a Nurse Instructor for our client, a certified home care agency, located in Brooklyn, NY. Schedule: Monday - Friday 8:30 am - 5:00 pm. Permanent Position.

In this role you will take referrals and process appropriate home care services; as well as maintaining open lines of communication with referral sources and within the company departments.

Qualifications

• Active NYS RN license.
• Minimum 2 years of nursing or intake experience in a licensed home care agency (CHHA).
• Knowledge of Allscripts is required.
• EMR knowledge: HomeCare HomeBase (HCHB) or Careport.

Responsibilities

• Screen referrals for clinical appropriateness and payer eligibility.
• Triage new referrals and identify urgent clinical needs.
• Review medical records, hospital summaries, and current medications.
• Verify and obtain physician orders and prior authorizations as needed.
• Complete timely intake documentation in the EMR.
• Coordinate admission scheduling with patient/family, clinicians, and physicians.
• Communicate referral details to the interdisciplinary team.
• Educate patient/family on home health services, visit expectations, and patient rights.
• Identify and escalate safety risks, infection control issues, or barriers to care.
• Arrange or schedule initial assessments (telephonic or in-person).
• Maintain compliance with Medicare, state, and agency admission requirements.
• Track referral outcomes, denials, and report trends to management.
• Provide telephonic triage or on-call handoff for intake issues (if applicable).
• Train/support intake staff on screening and documentation standards
