New York Community Care in the Bronx is seeking a full-time Intake Coordinator who speaks Spanish to join our in-person intake team. The role processes referrals, verifies insurance, educates patients on services, and helps determine approved care needs.

The ideal candidate has knowledge of HHA Exchange and at least 1 year of intake experience. We offer health insurance and paid time off and a Monday–Friday schedule.

Qualifications

• Must have knowledge of HHA Exchange.
• 1 year intake experience preferred.
• Spanish speaking required.

Responsabilités

JOB RESPONSIBILITIES INCLUDE:

Receive referrals from referral sources. Processing referrals received from marketing sources such as hospitals, nursing homes, and community physicians.

Verify patient’s insurance to determine eligibility

Educate patients on services we provide

Determine what services were approved by insurance

Contact patients to educate them on the services we can provide and assist them with their Home Care needs for skilled and non-skilled services.
