You will collaborate with referral sources, MLTC plans, Medicaid resources, and internal teams to advance each case while maintaining accurate records and timely communication.

Qualifications

• Minimum 1 year of home care intake experience.
• Understanding of MLTC requirements and home care intake processes.
• Experience with Medicare/Medicaid or MLTC insurance preferred.
• Strong written and verbal communication skills.

Responsibilities

As a Home Care Intake Coordinator, you will play a critical role in processing patient referrals and ensuring a smooth transition into home care services. You will receive and process referrals, contact patients and families to explain services and gather required information, verify insurance and determine eligibility, and educate families on home care services. Requirements

• Minimum 1 year of home care intake experience
