Job summary

Castle Rock Home Care in New York City seeks a motivated Home Care Field Marketer to develop and maintain referral sources across NYC. You will build relationships with healthcare professionals, discharge planners, case managers, and community organizations to generate client referrals for home care services.

You will navigate Medicaid and MLTC plans, educate partners on LHCSAs, and travel daily across the five boroughs while meeting referral targets.

Qualifications

• 1+ year of healthcare marketing, home care, or medical sales experience (NYC experience strongly preferred).
• Existing relationships with MLTC plans, hospitals, or nursing facilities is a major plus.
• Knowledge of Medicaid home care services and NYC referral processes.
• Strong interpersonal and networking skills.
• Self-motivated with the ability to work independently in the field.

Responsibilities

• Develop and maintain relationships with referral sources including hospitals, rehab centers, nursing homes, and social workers.
• Promote home care services to discharge planners, case managers, and community organizations.
• Educate referral partners on LHCSA services offered.
• Generate and track new patient referrals and admissions.
• Conduct in-person visits across NYC boroughs (Brooklyn, Queens, Bronx, Manhattan, Staten Island).
• Represent the agency at community events, health fairs, and senior centers.
• Collaborate with intake and coordination teams to ensure smooth patient onboarding.
• Maintain accurate records of outreach activities and referral pipelines.
• Ensure all marketing practices comply with HIPAA and NYS DOH regulations.

This role focuses on building relationships with healthcare professionals, community organizations, and facilities to generate client referrals for home care services.

The ideal candidate understands the NYC healthcare landscape, including Medicaid and Managed Long-Term Care plans, and is comfortable working in a fast-paced, target-driven environment.

Key Responsibilities

• Develop and maintain relationships with referral sources including hospitals, rehab centers, nursing homes, and social workers
• Promote home care services to discharge planners, case managers, and community organizations
• Educate referral partners on services offered through Licensed Home Care Services Agencies (LHCSAs)
• Generate and track new patient referrals and admissions

• Collaborate with intake and coordination teams to ensure smooth patient onboarding
