Job summary

Society's Assets, Inc. seeks a Home Care Director to provide leadership to the Home Care Department serving Racine, Kenosha, Walworth, Jefferson, and Rock counties. This is a full-time, on-site role with typical hours Monday-Friday, 8:00 AM-4:30 PM and an after-hours on-call rotation.

Requirements include Wisconsin RN licensure, at least one year of supervisory experience in personal care, and proficiency with MS Office, email, and data entry. Travel across 5-county service areas is required.

Qualifications

• RN license in Wisconsin (DHS S.50.49; DHS 105.17) required.
• Minimum 1 year supervisory/administrative experience in personal care or related program.
• Proficient with basic computer tools: email, MS Office, data entry, online communication.

Responsibilities

• Mentors and supervises all Home Care Department team members.
• participates in the after-hours on-call rotation and maintains on call calendars.
• Oversees delegation of medically oriented tasks to unlicensed assistive personnel (UAP).
• Travel to consumers for new visits and required supervisory visits every 50-60 days as needed.

Job description

The Home Care Director will provide leadership and direction to the Home Care Department that provides services in Racine, Kenosha, Walworth, Jefferson, and Rock Counties. This is a Full-Time, On-Site position, typical hours are Monday-Friday 8:00AM-4:30PM and sharing after hours on-call rotation.

Required Qualifications:

• Be licensed as a Registered Nurse in Wisconsin pursuant to DHS S.50.49 and DHS 105.17.
• Have at least one year of supervisory or administrative experience in personal care services or in a related program.
• Basic computer skills, including email, Microsoft Office, data entry, and online communication tools.
• Reliable transportation and ability to travel across 5-county service areas (Racine, Kenosha, Walworth, Jefferson, and Rock counties).

Key Responsibilities: (include but not limited to)

1. Leadership & Administration

• Mentors and supervises all Home Care Department team members.
• Participates in the after-hours on-call rotation and maintains on call calendars.

2. Clinical Oversight & Training

• Oversees the delegation of medically oriented tasks to unlicensed assistive personnel (UAP).

3. Regulatory Compliance & Quality Assurance

• Serves as the primary resource for Wisconsin Medicaid Personal Care regulations and Prior Authorization (PA) processes.
• Be knowledgeable and keep current about Federal and State and the Department of Health Services as applicable to Personal Care, Supportive Home Care, including but not limited to: DHS 105.17, 107.112, 133, the Medicaid Personal Care Handbook, EVV, and other regulations as required.
• Travel to consumers for new consumer visits and required supervisory visits every 50-60 days as needed.
