Job summary

PACE North in Traverse City, MI is seeking a Home Care Coordinator to assess participants' home care and safety needs, and to develop individualized plans with the IDT. You will coordinate services to support older adults living safely at home and communicate with families and care providers to ensure high-quality care.

This full-time, in-person position requires CPR certification and a Michigan driver license, plus at least 1 year of experience with frail or elderly populations.

Qualifications

  • Education: High School Diploma or equivalent; associate degree preferred.
  • CPR certification and Michigan driver's license required.
  • Minimum 1 year of experience with frail or elderly population.
  • Compassionate, organized, and strong communicator.
  • Proficient with basic computing and EMR systems.

Responsibilities

  • Conduct initial and ongoing assessments of participants' home care and safety needs.
  • Develop and update individualized home care plans with the IDT.
  • Coordinate home care services, supplies, and equipment to support at-home living.
  • Monitor participant status and adjust services as needs change.
  • Communicate with participants, families, caregivers, and care providers to ensure quality care.
  • Coordinate care across home, assisted living, and nursing home settings.
  • Maintain accurate EMR documentation and ensure regulatory compliance.
  • Participate in quality improvement initiatives and program development.
  • Promote participant rights, dignity, and quality of life.
  • Travel to participant homes and community facilities as needed.

What is PACE North?

PACE stands for Program of All-Inclusive Care for the Elderly. We are a trusted option for individuals 55 and older who qualify for nursing facility level of care and wish to remain in their homes as they age. What does that mean? We care for the elderly daily both at our state-of-the-art day center on Garfield near Hammond during regular business hours Monday through Friday—and as needed in their homes.

What does a Home Care Coordinator do at PACE North?
  • Conduct initial and ongoing assessments of participants' home care and safety needs.
  • Develop and update individualized home care plans in collaboration with the Interdisciplinary Team (IDT).
  • Coordinate and oversee home care services, supplies, and equipment to support participants living safely at home.
  • Monitor participant status and adjust services as needs and conditions change.
  • Communicate regularly with participants, families, caregivers, and care providers to ensure quality care and address concerns.
  • Coordinate care across home, assisted living, adult foster care, and nursing home settings.
  • Maintain accurate, timely documentation in the electronic medical record (EMR) and ensure regulatory compliance.
  • Participate in quality improvement initiatives, audits, and program development activities.
  • Promote and protect participant rights, dignity, independence, and quality of life.
  • Travel to participant homes and community facilities as needed.

Please note: This is a full-time, in-person opportunity based in Traverse City, Michigan. Home visits and occasional off-schedule or weekend work may be required based on participant care plans.

Requirements
What qualifications should you bring to the table?
  • Education: High School Diploma or equivalent required; Associate degree or college coursework preferred.
  • Licensure/Certification: Valid CPR certification and Michigan Driver License with current auto insurance and acceptable driving record.
  • Experience:
