Qualifications

• RN license is required for the role.
• BSN preferred for preferred qualification.
• 1-3 years of relevant Home Care/Hospice/Community Health experience required.

Responsibilities

• Supervise Home Care Nurses, Physical Therapists, Occupational Therapists, Speech and Language Pathologists, and Dietitians; review cases and records.
• Coordinate clinical and leadership activities with the Director of Home Care.
• Attend case conferences and interdisciplinary meetings to discuss patient progress and plans of care.
• Provide guidance to clinicians to improve performance and quality of services.
• Participate in on-call rotation and act as a liaison among staff, patients, and the Agency.
