Eminence Home Care is seeking compassionate Home Care Caregivers/Direct Care Workers with driving capabilities to join our team. You will provide exceptional in-home care, assist clients with transportation needs for medical appointments and social activities, and collaborate with families and health professionals to implement tailored care plans.

Qualifications

• High school diploma or equivalent required; additional caregiving certifications are a plus.
• Valid driver’s license with a clean driving record; reliable vehicle required.

Responsibilities

• Assist clients with daily living activities such as bathing, grooming, and meal preparation.
• Provide companionship and emotional support to enhance clients' well-being.
• Monitor and document clients' health status, reporting any changes promptly.
• Administer medications as directed and comply with care plans.
• Drive clients to medical appointments, errands, and social activities safely.
• Maintain transportation logs and vehicle cleanliness and safety.
• Communicate with clients, families, and healthcare professionals about needs and progress.
• Perform light housekeeping to maintain a safe, clean environment.

In this specialized role, you will provide exceptional in-home care while also assisting clients with transportation needs, ensuring they attend medical appointments, social outings, and other essential engagements safely and comfortably. Your role is critical in promoting independence and improving the quality of life for our clients by delivering personalized daily living assistance, companionship, and emotional support. You will collaborate closely with clients, their families, and healthcare professionals to implement tailored care plans with professionalism, empathy, and respect.
