Position Summary
This position provides personal care and/or home management tasks to clients requesting and eligible for Caregiver services, under the direction of the RN.
Essential Duties & Responsibilities
- Practice within the scope of training, license, certification, or registration, where applicable, and as required by law and regulation.
- Perform tasks as delegated and directed by the registered nurse.
- Assist clients with Activities of Daily Living and/or Instrumental Activities of Daily Living, including but not limited to:
- Assisting with ambulation, transfers, or exercises
- Assisting with bathing, dressing, and grooming
- Assisting with feeding
- Assisting with toileting and incontinence care
- Housekeeping
- Meal preparation
- Safe transportation
- Medication reminders
- Pet care
- Maintain general order, safety, and cleanliness of client area.
- Communicate any changes in client condition or occurrences to the registered nurse.
- Maintain chart documentation of care provided in accordance with the written plan of care.
- Practice safety and infection control methods.
- Demonstrate proper transfer and lift techniques using good body mechanic techniques.
- Maintain confidentiality & privacy of any PHI per HIPAA requirements.
- Exhibit competency in the observance of Universal Precautions by:
- Knowing standard precautions
- Washing hands before and after patient care
- Wearing gloves and other PPE as required

Minimum Qualifications
Experience
- Caregiving experience preferred
Licensures & Certifications (if applicable)
- Valid driver’s license and proof of automobile insurance
Summary Of Physical Requirements
- Walking, standing, sitting, bending, reaching, twisting, crouching/squatting, kneeling
- Flexion/Extension of arms/wrist
- Hand dexterity to include gripping, pushing/pulling, and writing
- If assigned to hands-on client, ability to assist with transfers/repositioning
- If assigned to hands-on client, lifting/carrying of 50 lbs
