Job summary

University Hospitals in Portage County is seeking a Home Care Assistant (STNA) for part-time coverage. You will provide patient care under supervision of a Case Manager—Nurse or therapist, and support plans of care in patients' homes.

The role requires CNA certification, BLS within 30 days, a valid Ohio driver's license with insurance, and at least 1 year of home care experience preferred. You will travel 100% to patients' homes and complete required agency tests.

Qualifications

• CNA certification is required.
• BLS certification must be obtained within 30 days of hire.
• OH Driver's License with car insurance is required.
• 1+ years of home care experience preferred.

Responsibilities

• Provides patient care, under supervision of a Case Manager: Nurse or therapist.
• Provides personal care/companionship as directed by plan of care
• Provides light housekeeping/light meal prep tasks as directed by plan of care
• Effectively communicates both verbally and in writing
• Initiates and maintains positive relationships with patients/customers and co-workers
• Observes and reports patient status changes including skin and psychosocial
• Takes responsibility for self-development and supports a learning environment
• Displays commitment to the mission of UHHS/UHHCS and its values
• Performs other duties as assigned.
• Complies with all policies and standards.
• Must abide by all requirements to safely and securely maintain PHI for our patients.
• Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
• For specific duties and responsibilities, refer to documentation provided by the department during orientation.
• 100% travel to the patients homes for care.

• Part time 24 hours per week.
• Monday-Friday.
• This position will cover Portage County.

100% travel to the patients homes for care.

• High School Equivalent / GED required and
• Completes required continuing education requirements to maintain certification required.
• 1+ years Home care experience preferred.
• Certified Nursing Assistant (CNA) or equivalent certification required and
• Basic Life Support (BLS) Required within 30 Days and
• OH Driver's License (Valid) with car insurance coverage required.
• Successful completion of agency written test and competency evaluation of skills in the field Required within 30 Days.
