The Home Care Assistant provides residents/clients and families with the highest quality of home care services, including companionship, personal care, transportation, and home support, as well as a wide variety of ancillary services depending on the specific needs and service plan of the client. Delivers specialized care following individualized training and competency criteria, in accordance with the Pennsylvania Department of Health guidance. All services listed in the service plan are provided, including any additional services requested if allowed by state regulations and Aspire for Well-Being Home Care policies. Services are provided in a caring and compassionate manner that recognizes individual needs, preferences, and rights.

The content of this job description is intended to describe the general nature and level of work expectations performed by incumbents in this classification.

Essential Functions:

1. Provides companionship and home support services to promote resident/client independence, including light housekeeping, laundry, linen changes, meal preparation, errands, grocery shopping, medication management, and appointments.
2. Assists residents/clients with activities of daily living, such as bathing, dressing, toileting, grooming, oral hygiene, skin care, and ambulation.
3. Provides specialized care as per individualized training and competency, following Aspire for Well-Being Home Care policies.
4. Provides transportation as listed in the service plan and/or at the resident’s/client’s direction, following policies and procedures.
5. Observes and reports any changes in the physical or mental/emotional state of residents/clients to the supervisor promptly.
6. Demonstrates good time management and engagement throughout the shift, providing additional support where possible.
7. Works independently with good judgment.
8. Reviews previous shift notes and documents services provided during the shift.
9. Provides updates to supervisors regularly and shares resident/client information as needed.
10. Coordinates tasks with other staff to ensure seamless service delivery.
11. Maintains respectful and courteous interactions with residents, families, visitors, and staff.
12. Protects residents’/clients’ privacy, confidentiality, and dignity.
13. Follows all policies and procedures, and suggests improvements.
14. Adheres to safety practices to ensure health and safety of all, including proper use of equipment and supplies.
15. Completes skills assessments and annual competency evaluations.
16. Communicates professionally about business-related issues.
17. Uses appropriate systems to log shifts and document services.
18. Completes all required forms accurately and honestly.
19. Attends mandatory training and demonstrates a commitment to quality improvement.
20. Participates in on-call rotations if designated, and is compensated accordingly.

Position Details:

This is an as-needed/per diem position with no guaranteed hours. Availability during evenings, weekends, holidays, and inclement weather is required. Travel may be required for client services.

Qualifications:

• High School Diploma or GED preferred.
• Certification as a Nursing Assistant is optional.
