Job summary

Nurse Next Door Weatherford/Granbury is seeking a Personal Care Attendant or Home Health Aide to join our in-home care team in Texas. You will work one-on-one with patients, following a tailored care plan in their homes, delivering compassionate, personalized care with attention to detail and clear communication.

You should be reliable, organized, and capable of building trust with patients and families. Prior Home Health or Hospice experience is a plus, and a valid driver's license and

Responsibilities

• Assist patients with daily activities (bathing, mobility, transfers).
• Perform household tasks related to patient care (light housekeeping, laundry, meal prep).
• Maintain records of patient care and report observations to supervisor.
• Engage patients in conversation or reading to support mental alertness.
• Encourage patients through exercises or activities to promote activity and well-being.

Job Summary

We are looking for a Personal Care Attendant or Home Health Aide to join our team! You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
• Engage patients in exercises or other activities
