Job summary

FRESH START-NEW BEGINNINGS Home Care in Wake Forest, NC seeks a Home Care Aide to work one-on-one with patients in their homes, following a personalized care plan, and delivering attentive, compassionate support. Prior Home Health or Hospice experience is a plus, but training is available. You will maintain patient records, remind about medications, and help with daily activities to support

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
• Provide medication reminders
• Ensure safety of clients
