Job summary

Concoh Health Care Llc is hiring a Home Care Aide to work directly with patients in their homes, following a personalized care plan. You will build genuine relationships, stay organized, and communicate clearly to meet patient needs.

Previous Home Health or Hospice experience is a plus, and you will assist with daily activities, maintain records, and help create a healthy living environment for patients.

Qualifications

• Strong verbal and written communication skills are required.
• Must have a Home Health Aide program accreditation.
• High School Diploma or GED is required.
• One year of professional experience is preferred.
• Driver’s license is required.
• CPR certification is required.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles.
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care.
• Maintain records of patient care, condition, progress, or problems to report to the supervisor.
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert.
• Give medications and immunizations.
• Engage patients in exercises or other activities.
