Home Care Aide
Position Summary
Currently hiring Home Care Aides for per diem hours in Bucks, Montgomery, and Philadelphia Counties and surrounding areas. Provides personal care and assistance to Support Services clients under the supervision of the RN Clinical Supervisor.

Job Description: Provide quality care, primarily by assisting consumers with activities of daily living (ADLs), including dressing, feeding, bathing, grooming, oral care, and ambulating (including Hoyer Lift).

Key Qualifications:

• High School Diploma or equivalent.
• Certified HHA/CNA preferred; qualified under PA state regulation or able to pass skills and written tests for level HCA.
• Home care or home health experience required.
• Current driver's license and access to a car.
• Ability to read and understand written instructions.
• Ability to take direction via telephone in patient's home.
• Ability to observe and report changes in patient condition with a great attitude.
• Flexibility with hours.
• Clearances (background checks, State, FBI, Childline) required.
• Verifiable references.
• Ability to anticipate consumer needs.
• Passionate, creative, self-motivated, and proactive.
• Effective interaction with consumers, relatives, and staff.
• Strong planning, organizational, and detail-oriented skills.

Responsibilities/Essential Functions:

1. Observe and report safety hazards or significant observations regarding the consumer.
2. Complete 12 hours of annual in-services.
3. Maintain consumer confidentiality and adhere to HIPAA and agency policies.
4. Follow written and oral instructions.
5. Report abnormal findings to RN as per care plan.
