Home Care Aide

Position Summary

Currently hiring Home Care Aides for per diem hours in Bucks, Montgomery, and Philadelphia Counties and surrounding areas. Provides personal care and assistance to Support Services clients under the supervision of the RN Clinical Supervisor.

Job Description

Provide quality care, primarily by assisting consumers with activities of daily living (ADL's) including dressing, feeding, bathing, grooming, oral care, and ambulating (including Hoyer Lift).

Key Qualifications

• High School Diploma or equivalent.
• Certified HHA/CNA preferred; qualified for employment under PA state regulation or able to pass skills and written tests for level HCA.
• Must have home care or home health experience.
• Current driver's license and access to a car.
• Ability to read and understand written instructions.
• Ability to take direction via telephone in patient's home.
• Ability to observe and report changes in patient condition.
• Great attitude and flexibility with hours.
• Clearances: background checks, State, FBI, Childline.
• Verifiable references.
• Ability to anticipate consumer needs.
• Passionate, creative, self-motivated, and proactive.
• Effective interaction with consumers, relatives, and staff.
• Good planning, organizational, and detail-oriented skills.

Responsibilities / Essential Functions

• Observe and report safety hazards or significant changes in consumer's condition.
• Complete 12 hours of annual in-services.
• Maintain consumer confidentiality and adhere to HIPAA and agency policies.
• Follow written and oral instructions.
• Report abnormal findings to RN as per care plan.
