Job summary

A New Leaf seeks a Family Connections Consultant in Mesa, AZ to provide home-based support to families involved with the Department of Child Safety. You will strengthen protective factors, enhance caregiver capacity, and promote child safety through trauma-informed, culturally responsive interventions.

The role requires a Bachelor of Social Work or related degree and at least one year in human services. Experience with multi-problem families and knowledge of community resources are essential;

Qualifications

• Bachelor of Social Work or a related bachelor’s degree required.
• Minimum of one year of experience in the human services field.
• Experience with multi-problem families and complex family systems.
• Knowledge of family dynamics and family-centered service approaches.
• Knowledge of community resources and ability to assist families in accessing services.
• Experience with families affected by substance abuse, abuse/neglect, trauma, or related challenges.
• Experience in child welfare, family preservation, or related programs preferred.

Responsibilities

• Work directly with families involved with the Department of Child Safety to support child safety and family stability.
• Manage an assigned caseload and ensure timely completion of services, documentation, referrals, and follow-up activities.
• Conduct home visits and community-based meetings to assess family needs and progress.
• Provide trauma-informed, family-centered interventions and support related to parenting, mental health, and substance use.
• Meet with families to provide consultation, education, and skill development for family functioning and safety.
• Assist with service planning, linkages, and transition to ongoing supports, including accessing resources.
