Job summary

Apex Rehab Group in Sullivan County is seeking Occupational Therapists and Physical Therapists to join our home care team. We offer flexible scheduling and a chance to make a real difference by delivering one-on-one therapy in patients' homes.

Licensed OT/PT/ST in New York are encouraged to apply. Home care experience is a plus, but not required. A valid driver’s license and reliable transportation are needed to visit patients across the service area.

Qualifications

• Must be licensed in NY as OT, PT or ST.
• Home care experience is a plus but not required
• Reliable transportation and valid driver’s license
• Strong communication skills and a positive attitude

Responsibilities

• Provide therapy services to patients in their homes
• Create and carry out personalized treatment plans
• Track patient progress and adjust care as needed
• Educate patients and families
• Complete simple, timely documentation
• Work independently with support from our clinical team
