Job summary

ViaQuest Psychiatric & Behavioral Solutions is seeking a licensed or eligible social work/counseling professional to provide therapy and case management within clients’ homes and community settings.

The role emphasizes delivering scheduled visits, updating treatment plans, and documenting progress notes. Travel across the assigned service area is required, with collaboration among case managers and psychiatric staff to ensure optimal client outcomes.

Qualifications

• Bachelor’s degree in Social Work or related field with one year of therapy experience.
• Master’s degree in Social Work or Professional Counseling.
• LSW/LISW/LISW-S, LPC, LPCC, LPCC-S, LMFT, LIMFT, LIMFT-S license or equivalent.
• Experience in therapy and counseling is preferred.
• Experience within a home setting is preferred.
• Valid driver's license and reliable transportation.
• Willingness to travel throughout assigned service area.

Responsibilities

• Providing patient visits as scheduled.
• Ensuring treatment plans are up to date for each client.
• Completing comprehensive and succinct progress notes.
• Communicating and following up as needed regarding the client's status.
• Working collaboratively with those providing case management and/or psychiatric services.
