Job summary

Alternate Solutions Health Network is seeking a Speech Therapist to provide home-based skilled services across the Brewster, Greentown, Massillon, Navarre, Canton, Portage, and Barberton area. You will work within an interdisciplinary team, delivering personalized therapy and education to patients in their homes.

The role requires a current license as a Speech-Language Pathologist, at least one year of ST experience in acute care, and a valid driver's license.

Qualifications

• Current license as Speech-Language Pathologist in the employer's state.
• Minimum one year of ST experience in an acute care setting.
• Home care experience preferred.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Develop an individualized Plan of Care and adjust as needs change.
• Monitor patient progress with home exercise programs and report to physicians and agency staff.
• Help minimize hospital readmissions by teaching preventative measures.
• Counsel patients and families on disease/injury and home care needs across all phases of therapy.
• Complete clinical documentation following agency protocol and Medicare guidelines.
• Follow agency policies and communicate schedule/availability changes to schedulers.
