Job summary

Avera Health in Estherville, IA is seeking a Home Health Clinician to direct clinical planning, application and follow-up of medical equipment for patients at home. You will ensure effective care planning and coordinate with the patient care team.

You will provide direct patient care, perform home visits, monitor respiratory equipment, and train caregivers on device use. A strong emphasis on safety and documentation is required.

Qualifications

• Bachelor’s or 1-2 years related experience and/or training.
• Driver's license valid and insurable by carrier.
• Active RN license in state of practice.
• Active Respiratory Therapist license or NBRC registration.

Responsibilities

• Directs planning of equipment needs for patients and high-tech setups; completes home assessment for ventilator patients.
• Provides direct patient care including assessment, pulse oximetry, weaning, trach changes, suctioning and caregiver training on high-tech equipment.
• Monitors PAP and ventilator performance; recommends respiratory management changes and documents care plans.
• Knowledge of all HME equipment and supplies; handles transactions and inventory actions.
• Assist in equipment servicing, maintenance and cleaning per manufacturer specs and infection-control standards.
• Conducts home visits to perform quality checks and educate customers on proper use and safety.
• Represents the company to referral services and stays updated on products and services offered at the location.
• Pulls equipment based on physician orders; maintains inventory control to minimize loss.
• Acts as resource to staff and handles billing/reimbursement inquiries; supports site manager on special projects.
