St. Coletta's of Illinois is seeking a Home Based Services QIDP to join the Social Services team. This position is responsible for developing comprehensive services that support individuals to achieve their personal outcomes, coordinating services, overseeing the implementation of Service Agreements, and providing general case management to ensure individuals with intellectual and physical disabilities attain personal goals.

Schedule

This position is on-site Monday - Friday 8:00 AM - 4:30 PM.

Pay Range

The salary for the position is $44,000 - $52,000, depending on certification and experience level.

Responsibilities
  • Develop, monitor, and document Service Agreements, Chronological Logs, and Implementation Strategies (IS).
  • Explain all rights to individuals as required by DHS.
  • Review service agreements monthly, document service implementation, progress, and needs for modification.
  • Provide copies of service agreements to individuals and community support team members.
  • Ensure compliance with standards and regulations such as CARF, DMH/DD.
  • Advocate for services and supports to help the consumer achieve their outcomes.
  • Ensure consumer participation in all decisions affecting their lives.
  • Serve as primary contact for guardians and service providers.
  • Work with Independent Service Coordinator to facilitate Personal Plans, discovery, and CST meetings.
  • Complete additional assessments timely.
  • Develop Service Agreements that incorporate all needed services.
  • Evaluate services and treatment plans for appropriateness and compliance.
  • Train families, guardians, and personal service workers on service agreement implementation.
  • Complete service agreements and implementation strategies for tracking outcomes.
  • Complete monthly reviews of the consumer's Chronological log and submit hours of service in a timely manner.
  • Ensure all services are delivered as outlined in the service agreement.
  • Complete home visits in person or virtually as required.
  • Attend CST meetings for discharge planning when invited.
  • Complete documentation reviews as required by Quality Assurance.
Qualifications
  • Must be 21 years of age.
  • Must be a DHS certified QIDP or willing to become certified.
  • Must have at least one year of experience working with the ID/DD population.
  • Bachelor's Degree in Human Services or related fields.
  • Two years of Self-Directed Assistance experience preferred.
  • Pass criminal background check and physical/drug test.
  • Valid driver’s license.
