This position is responsible for the development of comprehensive services that support individuals to achieve their personal outcomes, coordination of services, overseeing the implementation of Service Agreements, and providing general case management to ensure individuals with intellectual and physical disabilities attain personal goals.

Details
  • Schedule: On-site Monday - Friday, 8:00 AM - 4:30 PM
  • Pay Range: $44,000 - $52,000 (depending on certification/experience level)
Responsibilities
  • Develop, monitor, and document Service Agreements, Logs, and Implementation Strategies
  • Explain rights to individuals as required by DHS
  • Review service agreements monthly and document progress and modifications
  • Provide copies of service agreements to individuals and their support team
  • Ensure compliance with CARF, DMH/DD standards, and regulations
  • Advocate for services and supports to help clients achieve outcomes
  • Ensure consumer participation in decision-making
  • Serve as primary contact for guardians and providers
  • Work with Independent Service Coordinator for planning meetings
  • Complete assessments and develop comprehensive service plans
  • Evaluate and ensure services meet individual needs and regulatory standards
  • Train families and staff on service agreements
  • Track individual outcomes and complete documentation reviews
  • Ensure services are delivered as outlined and conduct home visits as required
  • Participate in discharge planning and quality assurance reviews
Qualifications
  • Must be 21 years or older
  • Must be DHS certified QIDP or willing to become certified
  • At least one year of experience working with ID/DD population
  • Bachelor’s Degree in Human Services or related field
  • Preferred: Two years of Self Directed Assistance experience
  • Pass criminal background check, physical, and drug test
  • Valid driver’s license
