Job summary

Interim HealthCare Inc. is seeking a Home Health Physical Therapist to provide patient-centered in-home therapy, improving strength and mobility. You will work with a home health team to implement the plan of care and document progress, delivering hands-on therapy and patient education.

Requirements include an accredited PT program graduate and Massachusetts license, CPR certification, and at least 1 year in home health. The role offers flexible assignments and mileage reimbursement.

Qualifications

• Graduate of an accredited Physical Therapist program with Mass license.
• Minimum of 1 year of physical therapy experience, ideally in home healthcare.
• CPR certification and knowledge of home health regulations.

Responsibilities

• Provide home-based physical therapy to patients who have mobility and strength challenges.
• Collaborate with a home health team (RN, LPN, CNA, HHA, OT, SLP) on the care plan.
• Observe movements, document progress, and modify the plan as needed.
• Assist patients with exercises and hands-on therapy to improve mobility and reduce pain.
• Educate patients and families on the plan of care and safety during exercises.
