Qualifications

• Colorado licensure to practice medicine.
• Experience delivering home-based primary care.
• Ability to manage medically complex seniors.

Responsibilities

• Provide longitudinal primary care at patients' homes and facilities.
• Diagnose and treat chronic and acute conditions with team support.
• Address social determinants of health like housing, nutrition, caregiver support.
• Conduct specialty visits such as wound care and post-hospital care.
• Complete comprehensive evaluations including med reconciliation and fall risk.
• Use data systems to track outcomes and coordinate post-discharge visits.
• Collaborate with care teams to reduce readmissions and stabilize high-risk patients.
