Responsibilities include managing chronic diseases, performing home assessments, and educating patients. Candidates must have an active NP or PA license in Indiana and at least 2 years of experience in primary care. Qualifications

• Minimum 2 years of clinical experience as an NP/PA in primary care or home care setting.
• Unrestricted NP or PA license in the State of IN.
• Nurse Practitioner certification from ANCC/AANP.
• Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS).

Responsibilities

• Perform comprehensive in-home assessments.
• Manage chronic diseases and complete physical examinations.
• Educate patients on disease processes and medication compliance.
• Travel 5 days a week to provide care to patients in their homes.
