Qualifications

• 2-3 years of experience working with elderly population is required.

Responsibilities

• Deliver high-quality primary care directly to patients in their homes.
• Manage a smaller caseload of assigned primary care patients (150-185).
• No weekends or holidays with minimal on-call requirements.
