Job summary

Summa Health is seeking an IM or FM physician to join our Senior Health team in Akron, Ohio. The role focuses on home-based primary care for vulnerable seniors, with an emphasis on coordinated, patient-centered care.

The selected physician will support the evolution of Summa’s Complex Care Institute, collaborate with APPs, social workers, and pharmacists, and contribute to education of medical students, residents, and fellows. Employed position with comprehensive benefits and salary guarantee.

Qualifications

• Board Certified/Board Eligible in Internal Medicine or Family Medicine.
• Unrestricted Ohio License or willingness to obtain.
• Commitment to quality care in a patient centered, team-based environment.

Responsibilities

• Support home-based primary care in the community.

• Involve in medical student, resident, and fellow education within Summa Health.

Home Visit Primary Care Physician

Summa Health is seeking an IM or FM trained physician to join our growing Senior Health team. The chosen candidate will support our home-based primary care in the community.

The candidate will support the evolution of Summa’s Complex Care Institute, whose vision is ‘to meet people where they are and advocate for what matters most’. This team provides care to over 500 patients in the community. Additionally, they provide the most vulnerable and complex patients with comprehensive, interdisciplinary primary care and support in a cost effective, efficient, and well-coordinated plan.

The selected candidate will become a member of the Summa Health Medical Group, a multi-specialty group practice with nearly 700 providers employed by the Summa Health System. Comprised of 2 hospitals and over 20 health centers, Summa Health is one of the largest integrated Healthcare Delivery Systems in the state.
