Job summary

THERAPY 2000 in Texas seeks a dedicated pediatric Physical Therapist to provide home-based care to children and families. You will perform evaluations, design individualized treatment plans, and support patients in doing activities that promote growth and independence in the comfort of their own homes.

The role emphasizes collaboration with families and a team of providers, with bilingual English/Spanish communication and adherence to agency standards. Prior home-health experience is preferred.

Qualifications

• 3+ years pediatric PT experience.
• Experience in a home-health setting.
• Bilingual English/Spanish.

Responsibilities

• Perform comprehensive Physical Therapy evaluations using standardized tests.
• Develop and implement treatment plans aligned with physician orders.
• Develop home exercise programs with caregivers.
• Collaborate with other providers to coordinate care.
• Complete a minimum number of patient visits weekly as required.
• Meet deadlines for chart updates, progress summaries, and logs.
• Engage with agency personnel to align with expectations and standards.
• Operate in compliance with all policies, codes of ethics, and laws.
